Humana Dental Insurance Plans

 

Humana Dental Preventive Plus – 1000

  • Preventive Services – 3 Routine Oral Examinations per Calendar Year
  • Periodontal Cleanings – 4 exams per Calendar Year
  • Calendar Year Annual Maximum of $1000
  • $50 Individual Deductible / $150 Family Deductible
  • Deductible Does Not Apply To Preventive Services
  • No Waiting Period for Preventive and Basic Services*
  • Basic Services Covered at 80% after Deductible
  • Major Services are Not Covered under the Preventive Plus plan
  • Orthodontia Services are Not Covered under the Preventive Plus plan

Humana Dental Traditional Preferred – 1000

  • Preventive Services – 3 Routine Oral Examinations per Calendar Year

  • After you reach the annual maximum amount, you will receive 30 percent coinsurance on preventive, basic and major services for the rest of the year (excludes orthodontia)

  • Deductible Does Not Apply To Preventive Services
  • No Waiting Period for Preventive, Basic or Major Services for groups with 10 or more enrolled employees*
  • Basic Services Covered at 80% after Deductible
  • Major Services Covered at 50% after Deductible
  • Orthodontia Services are Not Covered – Members may receive a discount on non-covered services up to 20%

*Groups with less than 10 enrolled will have a 12 month wait on Major Services

  • Deductible Does Not Apply To Preventive Services
  • No Waiting Period for Preventive, Basic or Major Services for groups with 10 or more enrolled employees. 12 month waiting period for Orthodontia Services*
  • Basic Services Covered at 80% after Deductible

  • Orthodontia Services – Child Orthodontia – Covers children through age 18. Plan pays 50% (no deductible) of the covered orthodontic services up to $1000 lifetime orthodontia maximum.

Humana has offered multiple Vision plans designed to allow employees to take advantage of routine eye exams and benefits.
